This position exists to develop, implement, and provide pharmaceutical care in all aspects of the clinical pharmacy department as part of a one year program. The Pharmacy Resident will optimize drug therapy outcomes by providing high quality, cost-effective health care; influencing prescribing behaviors; identifying, preventing and resolving drug-related problems; and educating health care personnel and patients regarding drug therapy.
Essential Functions:
Design, coordinate, and implement clinical programs to improve patient care, including but not limited to: pharmaceutical care clinics, therapeutic drug monitoring programs, therapeutic guidelines, and programs to support national and regional clinical initiatives
Provide pharmaceutical care for patients with direct supervision of referring physician with completion of appropriate documentation in electronic medical record
Apply appropriate techniques and strategies to effectively respond to drug information requests and critically evaluate medical and pharmacy literature
Assist in staffing established clinics that clinical pharmacy operates or supports, including but not limited to; anticoagulation clinic, pharmacy utilization management, pharmacy service center, cardiovascular risk service, and geriatrics clinic according to approved policies and procedures
Develop formulary recommendations in conjunction with medical staff. Present evaluations and recommendations to the P& T Committee and communicate and implement formulary decisions
Assist in teaching PharmD students on clinical pharmacy rotations from various schools of pharmacy
Qualifications
Basic Qualifications: No experience required
Graduation from an accredited school of pharmacy with PharmD (or B.S. in Pharmacy with equivalent clinical experience)
Licensed or eligible for licensure in the State of Ohio as a Registered Pharmacist
Exceptional communication (verbal and writing skills) as well as ability to establish and maintain working relationships with other health care providers with supervision
