Dispensing: Prepares and dispenses medication orders per physician request according to established policies, procedures and protocols.
Interprets medication orders (Verbal and written) and transcribes to computerized patient medication profiles accurately.
Issues controlled substances to patient care areas and maintains records as required by law.
Implement clearly defined process for medication distribution, handling and administration in the organization.
Monitoring: Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols.
Monitoring drug therapy regimens for contraindications, drug-drug interactions, and allergies.
Assesses patient for appropriateness of dose, route, and form of medication.
Assists with pharmacokinetic consult service and drug dosing on request.
Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.
Reports suspected adverse drug reactions to the Risk Manager.
Position specific competencies
Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy.
Provides accurate and adequate, and timely drug information to the professional staff.
Documents all clinical activities and interventions accurately and completely via department MUE program.
Assess, identifies and address, on an ongoing basis, the special needs of all ages of patients when providing patient care.
Screens drug appropriateness for age and physical systems tolerances.
Quality and efficiency: contributes to the quality and effective operation of the pharmacy.
Supervises and directs pharmacy personnel. Ensures in the performance appraisal of pharmacist and support personnel are completed according to the organization’s policy.
Develop and reviews P&P to ensure safe consistent and uniform level of care throughout the organization.
Current license as a Registered Pharmacist in good standing by the State of California.
Experience in acute care hospital or ambulatory pharmacy
Experience in Pharmacy management/supervision and management education
Demonstrated application of drug knowledge and clinical judgment.
Graduate of an Accredited School of Pharmacy
Physical Demands and Work Requirements:
Must be able to sit for 4 hours and stand/walk for 6 hours or more per day.
Must be able to reach overhead and lift/carry up to 50 pounds.
Moderate exposure to hazardous waste and materials.
