Description
PURPOSE: Under general supervision, provides customer focused pharmaceutical care and oversees the efficient dispensing, processing and recording of prescriptions issued by physicians, dentists, other authorized health care providers. Communicates with providers to influence optimal drug information to patients and health care providers. Participates in patient and provider educational programs to positively impact patient compliance and provider prescribing practices. Oversees and directs pharmacy technician staff in the execution of technical functions. Assists in cash and inventory control in accordance with departmental procedures and comply with all record keeping and legal requirement of the practice of pharmacy.
This position consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to the applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures. In addition, Regional leaders are accountable for communication, implementation, enforcement, monitoring and oversight of compliance policies and practices in their departments.
In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives. Also accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.
ACCOUNTABILITIES:
1. Analyses patient care and services needs in a manner that facilitates the achievement of both a high level of customer satisfaction and the attainment of organizational care and service initiatives.
2. Reviews prescribe medications and dosages to ensure that therapy is safe and effective for the patient's condition. Utilizes patient profiles to identify clinically significant drug allergies and drug interactions. Processes and labels prescriptions with proper identification data and directions for patient use. Compounds and dispenses medications accurately.
1. Provides advice and consultation to patients regarding side effects drug interactions, specific storage requirement and complete directions for use. Monitors designated patients' compliance and response to drug therapy to improve individual patient outcomes.
2. Routinely communicates with providers to influence the use of formulary medications in compliance with clinical protocols, target drug initiatives and pre-authorized therapeutic substitutions. Documents all clinical interventions using approved forms.
3. Demonstrates proficiency in operating automated systems for prescription order entry, patient profile review or retrieval of clinical or statistical data. Maintains proper records relating to medication profiles, prescription files, controlled substances records, non-formulary records and other legal and departmental record and procedures. Demonstrates proficiency with the Point Of Service (POS) cash register system and cash management procedures.
4. Serves as a liaison to assigned clinical care team as demonstrated by problem solving skills on medication related issues, input on education matters relating to medications and fostering interdepartmental coordination on pharmaceutical care initiatives.
5. Maintains professional competency through attendance and participation in clinical team meetings and ACPE accredited continuing education programs. Assists in the training and education of pharmacy students and technicians.
6. Participates in departmental quality improvement initiatives and contributes to team or departmental projects directed toward the achievement of improved service efficiency and patient outcomes.
MINIMUM REQUIREMENTS - Relevant Years of Experience:
1. Completion of an ASHP accredited pharmacy residency program desirable
2. Home IV experience preferred - IV experience highly preferred
3. At least one year experience as a pharmacist in an outpatient clinic, hospital or retail pharmacy desirable.
Education and/or Classes:
. BS in Pharmacy or Pharm.D. from an accredited college of pharmacy
License, Certification and/or Designation:
. Licensed in the applicable jurisdiction required, i.e., State of Maryland, Virginia, or District of Columbia as a registered pharmacist.
